A1C Frederick Smith, A1C William Urban, and Sergeant Jerome Ciereszewski, died when their helicopter experienced an in-flight malfunction and crashed. The pilot also died. The officers were en route to a facility to conduct a routine security check. The officers were members of the 862nd Security Police Squadron at Minot Air Force Base in North Dakota.
